The gameplay of ''The House of the Dead 4'' is somewhat different from the previous games, in that a submachine gun is used instead of the handguns of the first two, and the shotgun of the third. The game also features the use of a limited number of grenades that can be thrown to destroy large numbers of enemies.

In some points in the game, the player is required to actually shake his (or her) machine gun vigorously in order to escape certain situations such as being grabbed by a zombie. Unlike ''[[The House of the Dead III]]'', it has a wide variety of branching paths that is much more like the path system in ''The House of the Dead 2'', in that the player can take different paths that eventually lead to the same point in the game's story. The choices can be a point where you chose it, a shaking part to open a door, or shooting a special area to open something.

The most common enemies have three attacks. A normal attack, a grab and a push. A normal attack costs you a life, shaking causes a sequence that, if successful, pushes back oppenents for easier shooting. Pushing down, however, causes a difficult part if 3 zombies are on top of you as they stomp on you in about 5 seconds, causing damage to both players. Some other enemies have only one or two of these, needless to say they all have attack 1.

The story begins in 2003, three years after the events of ''The House of the Dead 2''. AMS agents James Taylor and Kate Green are gathering intelligence from the fifth basement floor of the AMS European branch office in London. For some reason, James suspects that the Goldman incident of 2000 had not yet ended. The room suddenly shakes violently, which partially collapses the basement, leaving the two agents trapped.

As a result, The World was hidden away underground and allowed to develop for three years following his death, before finally being released in the opening of "Pandora's Box" to fulfill its role as The Emperor's successor.

This creature, when it first appeared in the courtyard in front of the DBR office building, was visible only from the waist up. The World resembled a giant, crystalline, humanoid torso with an insectoid head, and had wing-like tendrils sprouting from its back. Although its weak point was unanalyzable, it was found to be a red, vertical slit housed in the center of its chest. The World had the ability to manipulate ice, shaping it into weapons, creating freezing shockwaves and flowers, and calling forth spiraling dragons.

Following its second defeat, The World evolved into a full humanoid, which prompted James to set his PDA to self-destruct, enabling him to sacrifice himself to destroy it. James' heroic sacrifice gave hope to humanity and Kate the resolve to never give up hope. And to continue to try to save humanity.
 
==House of the Dead 4 Special==
 
The House of the Dead 4 Special is a two-player attraction based off The House of the Dead 4. Players enter an enclosed room, sit down, buckle up, and experience zombie blasting like never before. The game makes use of two 100 inch screens, one in front and one in back, and a 5 speaker sound system, giving the impression that zombies are attacking from all sides. The seat shakes as zombies attack, and players are blasted with air for every bullet that penetrates their defenses. Sega's modified the game's light gun to make it more suitable for an attraction. Guns are now fully automatic and include a hand grenade button. This update to the Lindbergh debut gun shooter will follow a new side story in the House of the Dead plotline, teaming Kate Green up with "G".
