Vivian is a loup-garou, which is a werewolf, though that term is rarely used. It is a separate species from humans (they are incapable of breeding)that gains the power to transform into a wolf-like creature (the name is never mentioned, though it is said it is not a wolf) as the full moon approaches, it is easier the closer it is. On the night of the full moon, however, the loup-garou must transform, it is inevitable. The transformation can be held back for a few minutes or so, but it takes a toll on the loup-garou, often resulting in nausea and fatigue. They cannot reveal what they are, or face extinction. Silver kills them if it touches an open wound.

Vivian retains a generally wolf-like personality in human form, she is extremely stuborn and has no human friends. Meeting a boy named Aiden, she slowly falls for him. Gradually more members of her pack are introduced, and it is revealed that the other five young male loup-garou have been killing humans. This is forbidden by the Law of the Moon, any loup-garou that kills a human faces death. Vivian later reveals her true form to Aiden, who becomes frightened and forces her out through a window. Landing on her feet, the young loup-garou runs off. She wakes up in her own bed, with blood on her nails and no memory of what happened. In the news, a man was killed by a wolf-like animal. Simmilar incidents keep happening, until she finds a human hand on the floor of her bedroom. Vivian then decides to commit suicide for the sake of the pack, and just as she is about to light the match to the gasoline, the pack leader informs her that it wasn't her who killed the humans, but the five human-eating loup-garou who were tricking her.

They go along to a final showdown, where Aiden was tricked into comming. He shoots the leader of the five human killing wolves (Riku) with a silver bullet. Riku dies, then Aiden aims a gun at the pack leader. Vivian stands in front of him, but Aiden has already shot the gun. The pack's healer is summoned, who manages to get the silver bullet out fast enough to save Vivian's life, but she is trapped between her two forms.
Later, the pack leader enters her bedrom, and explains his own past with a human woman. The two loup-garou kiss, and Vivian is no longer trapped. He explains to her why this happened, that loup-garou want to love in both forms, so they begin to transform when fiercely kissed. The book ends with Vivian becoming the new alpha female.

Blood and Chocolate is also the name of a 1986 album by Elvis Costello and the Attractions. The album was the last recorded by the group before a breakup that would last for eight years, and reunited them with producer Nick Lowe. The album was, unusually for a "studio" album, recorded in a single large room at high volume, with the band listening to each other on monitor speakers rather than headphones. Costello describes it as "a record of people beating and twanging things with a fair amount of yelling". Costello is credited on the album under the pseudonym "Napoleon Dynamite".
