This index allows the reader of the novel to locate the page number where any of the book's 388 endnote numbers appear in superscript in the main body of the novel.

This is particularly useful to those who listen to the [[Hachette Book Group#Hachette Audio|Hatchett Audio]] audiobook, narrated by Sean Pratt, (ASINISBN B007SNQOSY9781611133868) because the endnotes themselves are not included. Instead, the voiced endnote number is inserted where it appears in the novel, and the reader must then consult the printed copy of the book for the endnote content, it which point it can be very useful to then see the context within the corresponding page; e.g.: '''375: 143, 144''' indicates that on '''Page''' '''375''' one will find '''Endnote 143 and 144'''.

N.B., pagination is the same in the hardcover book edition ('''ISBN 978-0-316-92004-9''', 1996) and the paperback, ('''ISBN 978-0-316-06652-5''', February 1997).
