In the last episode, Phil Leotardo is killed, while Tony seems to have hidden successfully. He comes out of hiding to spend time with his family in a restaurant in the last few minutes of the final episode. Several suspicious characters enter and Meadow has trouble parking her car. Just as she enters the restaurant, Tony looks up and it dramatically cuts to a silent black screen for a time before the credits are shown, leaving the ending open.
However, the ending could be representative of Tony's ultimate fate: he was whacked and everything just went to black.
Alternatively, the unresolved tension of the ending could be a metaphor for Tony's entire life, or the life of anyone who lives on the edge, wherein he must watch every door, be suspicious of every stranger and friend, and be concerned always for his family due to the mortal danger in which he has placed them -- until either he is "whacked", or he meets the ordinary fates of the mobsters he knows, e.g., cancer or Alzheimer's.
