Terminata la riabilitazione, tornò in [[World Championship Wrestling|WCW]] come "Lord Steven Regal". Come nell'esperienza precedente, Regal lottò principalmente in [[tag team]]. Continò ad interpretare il ruolo di [[heel (wrestling)|heel]] ed ebbe piccole faide con team come i [[The Revolution (WCW)|Revolution]] e i [[Filthy Animals]]. Ha una grande passione per gli animali e ha vari [[animali domestici]]. Nel 2003 gli fu mal diagnosticato un problema cardiaco, a causa del quale il lato destro del cuore non batteva in sincronia col sinistro: ciò determinò la formazione di una grande quantità di liquidi nel suo corpo. Dopo una serie di trattamenti medici, tornò a combattere nell'aprile del 2004.
