The example image lacks the elegance of contemporary web pages. This is obvious from the programming point of view, because the code is an unadorned HTML, but the unassuming reader may not appreciate this, and it may be useful (unless it is required to illustrate <em>history</em> of forms) to use a contemporary example styled with CSS and whatnot, maybe a fragment of an actual form in use somewhere prominent?

This article seems to almost exclusively deal with ''HTML'' web forms, but XForms are an alternative standard for expressing forms on the web, albeit poorly supported and rarely used.
