Once an image file has noise introduced into it, you basically have two choices... noisy or blurry. The most common method for "fixing" a grainy image is to apply some manner of smoothing filter to it. Noise is smoothed away (more or less), but now it's blurry. Personally I don't think that's an improvement so you won't find me doing it much. It's like painting a face on a horses ass. It's still a horses ass... only now it looks like someone tried to paint a face on it. So my advice is to either deal with the grainy photo... or look for a better one.
